To be eligible to receive equipment on loan, an applicant must:
1. be Hearing, Mobility, or Speech Disabled
2. be a resident of Montana
3. be able to demonstrate his or her ability to understand the nature
and use of the equipment for the purpose of sending or receiving
messages through the telephone system.
4. make less than 250 percent of the Federal Poverty level for the current year. Your annual family income must be included. To find out more

Approval or Denial of Application: Written notification of
approval or denial will be mailed within 30 days of receiving your
application. If the application is approved, another letter (or
possibly a telephone call) will list a location where you may receive
the equipment and the date, time and place of training session.
Training is provided in group sessions home visit only if
MTAP preapproved. If the application is denied, the letter
will state the reasons for denial and the process for appeal. If
the program sends three letters to schedule an appointment and receives
no response, then the application will be cancelled.
Certification: Eligibility is self-certifying. However, the
Program may request additional documentation on income or impairment
verification.
